The Diabetes Self-Management program (DSMP) is a community-based program that assists participants in managing type 2 diabetes. Participants make weekly action plans, share experiences and help each other solve problems they encounter in creating and carrying out their self-management program.

The Diabetes Self-Management Program (DSMP) is a six-week group program for people with type 2 diabetes and is designed to help participants manage their diabetes symptoms, tiredness, pain, and emotional issues, by helping them learn skills to better manage their diabetes day to day. DSMP is an evidenced based program developed at Stanford University and now managed by the Self Management Resource Center.
